OxyContin

OxyContin is a controlled-release tablet for the management of moderate to severe pain when a continuous, around-the-clock analgesic is needed for an extended period of time. OxyContin is not intended for use on an as needed basis. OxyContin has been associated with numerous reports of diversion and abuse, some resulting in death. The recommended dosing is one tablet every twelve hours. The limit is two tablets per day for all strengths (80mg is 4 tablets per day for high dosing needs).

Recognizing there may be certain patients who require higher dosing, an authorization process is available. Use the following form to request an exception:
