For 2008, HCPCS Q9965 (low osmolar material, 100-199 mg/ml iodine concentration) was added to the BCBSKS outpatient maximum allowable payment (MAP) listing for most hospital providers. This code is an add-on service however the BCBSKS MAP listing did not show that it was. Please correct our oversight by manually updating your MAP listing. If Q9965 appears on your list, please mark an "X" in the ADDON column. Thank you for making this correction. Drb
