HCPCS Q4098 – New Code and MAP Effective April 1, 2008

HCPCS code Q4098 is a new code effective April 1, 2008:

  • Q4098 – Injection, Iron Dextran, 50 mg

As allowed by the contracting provider Policies and Procedures, BCBSKS can establish payment guidelines for new codes.   In this case, there are two existing HCPCS codes for this drug that are currently subject to a maximum allowable payment.   They are:

  • J1751          Injection, Iron Dextran 165, 50 mg
  • J1752          Injection, Iron Dextran 267, 50 mg

Therefore, the MAP established for the new code, Q4098 is the same MAP that is currently applied to the existing codes, J1751 and J1752. (See your 2008 Outpatient Maximum Allowable Payments listing for the exact amount or contact the institutional relations department.)
