Modification of Home Health Type of Bill effective October 1, 2013 for Home Health Agency Claims
The National Uniform Billing Committee (NUBC) was formed to develop a single billing form and standard data set that could be used nationwide by institutional providers and payers for handling health care claims. The UB-04 Data Specifications Manual is the official source of data specifications adopted by the NUBC and contains codes used in the 837 Institutional electronic claims.
The NUBC has modified the UB-04 Form Locator (FL) 04 for the Type of Bill (TOB) for Home Health Agency claims. Effective October 1, 2013, TOB 33X will no longer be accepted for claims submitted on the UB-04 claim form. Claims submitted with a type of bill 33X will be denied for any claims received on or after 10/1/2013, even if the date of service is prior to October 1, 2013. Home Health providers must submit claims with one of the following TOBs effective October 1, 2013:
The following will be used for the 'X' in the 3rd digit of the TOB:

|3||Interim Billing – Continuing Claim|
|4||Interim Billing – Last Claim|
|7||Replacement of Prior Claim|
|8||Cancel Prior Claim|
