· Ambulatory Surgery Center

· Free-Standing Birthing Centers

· Free-Standing Cardiac Catheterization Laboratory

· Free-Standing Dialysis Centers

· Free-Standing Imaging Center

· Free-Standing Sleep Labs

· Home Health Agency, Hospitals

· Independent Laboratory

· Institutional providers

· Medical Care Facility which are authorized to diagnosis and/or treat nervous and mental condition

· Registered nurse

· Licensed practical nurse

· Supplier of orthopedic and prosthetic devices or medical equipment

Doctors and Professional providers as eligible if they fall under the following credentials:

• Licensed MD
• DO
• DDS
• Oral surgeon
• Podiatrist
• Chiropractor
• ARNP
• Optometrist
• Speech-language Pathologist
• Audiologist
• Registered Physical Therapist
• Occupational Therapist
• Physicians Assistant
• Psychologist
• Licensed Specialist Clinical Social Worker
• Certified diabetic educator/licensed dietician

Services which are provided by non-eligible providers may be denied as patient responsibility.
