Now that you have found a health plan, you will need to tell us about your business.
To receive a premium quote, you and any interested employees will need to complete a Health Profile, and provide information on medical history for the past five years.
If you are accepted for coverage, we will contact you within seven days with a guaranteed premium quote for health and optional dental and life insurance coverage. Your answers assist us in accurately determining premiums for coverage in which you are interested. Completion of the Health Profile in no way obligates you to purchase coverage.
This information is confidential and secure. Only authorized Blue Cross and Blue Shield of Kansas employees who will evaluate your health insurance coverage will have access to this profile.
If any information is incomplete the Health Profile will be returned to you, resulting in delays of the review process.
