Basic Blue

Basic Blue provides solid protection for basic hospital services, at an affordable cost. You choose the deductible that fits your budget. Once the deductible is met, you pay a 50% coinsurance to a maximum of $1,000 for each person ($2,000 two-or-more coverage). Then we pay 100% of allowed charges for the remainder of the benefit period. And you’ll receive a $5 million lifetime benefit maximum.

Deductible
A choice of deductibles is available:

• $500 per person/$1,000 two or more persons
• $1,000 per person/$2,000 two or more persons

Coinsurance
Expenses are covered at 50 percent up to $1,000 per person/$2,000 two or more persons, then BCBSKS pays 100 percent of allowed charges for the remainder of the benefit period.

Additional Program Information

• Inpatient Admissions - Pre-admission certification is required for all planned inpatient admissions.
• Dependent Coverage - Unmarried dependent children are covered to age 23.
• Benefit Period - The 12-month period based on the group anniversary month.
• Waiting Period - Businesses select a waiting period option.
