To begin the application process:

  1. Complete a Health Profile, where you provide information on your medical history for the past five years.
  2. If you are accepted for coverage, we will contact you within seven days with a guaranteed rate quote for health and optional dental coverage. Your answers help us to accurately determine rates for the plan in which you are interested. If any information is incomplete the Health Profile will be returned to you, resulting in delay of the review process.

This information is confidential and secure. Only authorized Blue Cross and Blue Shield of Kansas employees who will evaluate your health insurance coverage will have access to this profile. Completing the Health Profile in no way obligates you to purchase coverage.
