What is the disease management program?
Our disease management program is a one-on-one educational program conducted via the telephone with a registered nurse employed by Blue Cross and Blue Shield of Kansas. We offer this program to members, ages 21 to 63, who have BCBSKS as their primary insurance. With the exception of communicating with your treating provider, any information gathered during the course of the program will remain confidential.
Why does BCBSKS offer this program to me?
Our main goal is to help our members stay as healthy as possible. You will learn about self-management behaviors, plus the same registered nurse will support you with your health goals throughout your time in the program. You could go so far as to say that your registered nurse acts as your own personal health coach.
How much does this program cost and will my participation make my insurance costs go up?
Participation in our disease management program is provided at no additional cost, and will not affect your benefits.
Will the information I give about my health be shared with anyone else?
With the exception of communicating with your treating provider, any information gathered during the course of the program will remain confidential.
Does my doctor know about this program and do I need his approval to participate?
All contracting doctors in our service area have been educated about this program and are periodically reminded about how their patients might benefit. You do not need an okay from your doctor but we do encourage you to let your doctor know that you are participating in this program. This program does not replace your doctor appointments.
How do I get started?
Complete the online enrollment form, email email@example.com or call 1-800-520-3137 today. An enrollment specialist will set up your first telephone appointment with a registered nurse on a date and time that works best for you.
What happens after I sign up for my first appointment?
You will receive a save-the-date card and a packet of information about the program. The save-the-date card lists your appointment time and also provides a list of items you need to have with you during your first appointment.
How long will I be in the program?
You and your nurse will discuss your goals to determine your length of time in the program.
How will I remember all the information that the nurse and I talk about?
Soon after the phone call, your nurse will mail you educational handouts about the topic that was discussed during the phone call.
How will I remember when the nurse is calling next?
At the end of each phone call, you and the nurse will set the next call on a date and time that works for you. You will get a reminder postcard in the mail one week before the call to help you remember the date and time.
I am interested in joining your disease management program but do not have time to speak with a nurse during the work day, do you offer evening hours?
Yes, our registered nurses also are available to speak with you between 5 and 9 p.m., Mondays, Tuesdays, Wednesdays and Thursdays.
