Dental Insurance for Groups with 9 or Fewer Employees

Employees, especially those with families, value dental benefits. You can expand our coverage to include dental services – our seven options provide comprehensive coverage for most preventive services. Benefits for all services are paid at the maximum payment allowed.

Option 1: Dental Care Program

50% coverage for services such as basic check ups, dental X-rays (limited) and cleanings.
50% coverage for crowns, bridges and dentures.

Option 2: BuildingBlock Dental PPO

50% coverage for oral examinations, inlays, sealants and more.
50% coverage for oral surgery (consisting of diagnosis and treatment of fractures, dislocations, cysts and abscesses; and surgical extractions), space maintainers, and onlays and crowns that are not part of a bridge.
50% coverage for dentures, bridges and dental implant services ($1,000 lifetime max per insured, per arch).
50% coverage for surgery of the bony structure supporting the teeth, periodontic treatment of the gums, consisting of examination, management and surgery.

Option 3: BlueCare DentalPlus Includes teeth whitening!

Pays $200 annually for whitening services and supplies provided by a dentist.
50% for major services following a 12-month waiting period; subject to a $50 annual deductible per person.
80% for primary services; subject to a $50 annual deductible per person.
Maximum benefit payment of $1,500 per benefit period.

Option 4: BlueCare DentalPlus PPO

Lower monthly premiums than our standard BlueCare DentalPlus rates.
Save a standard 15 percent on all dental service costs when using an in-network dentist.
Pays $200 annually for whitening services and supplies provided by an in-network dentist. Coverage for whitening services is not available out of network.
50% for major services following a 12-month waiting period; subject to a $50 annual deductible per person.
80% for primary services; subject to a $50 annual deductible per person.
Maximum benefit payment of $1,500 per benefit period.

Option 5: Comprehensive Dental

100% coverage for basic check ups, dental X-rays (limited) and cleanings.
Other covered services (including simple extractions, fillings and repair of dentures) covered at 80% after meeting $25 per person / $75 per family deductible. Maximum benefit payment is $1,500 per person each benefit period.
50% coverage for major services such as dental implant services and periodontal surgery.
50% coverage for crowns, bridges and dentures.

Option 6: Comprehensive Dental

100% coverage for basic check ups, dental X-rays (limited) and cleanings.
Other covered services (including simple extractions, fillings and repair of dentures) covered at 50% after meeting $25 per person / $75 per family deductible. Maximum benefit payment is $1,500 per person each benefit period.
50% coverage for major services such as dental implant services and periodontal surgery.
50% coverage for crowns, bridges and dentures.

Option 7: Comprehensive Dental

100% coverage for basic check ups, dental X-rays (limited) and cleanings.
Other covered services (including simple extractions, fillings and repair of dentures covered at 80% after meeting $50 per person / $150 per family deductible. Maximum benefit payment is $1,000 per person each benefit period.
50% coverage for major services such as dental implant services and periodontal surgery.
50% coverage for crowns, bridges and dentures.
