Provider attestation change; updated form

In order to maintain accurate, high-quality provider information and ensure compliance with accreditation standards, additional questions will be added to the provider attestation form in the Provider Portal effective April 4, 2026. These questions are required as part of the attestation process and must be completed to submit the attestation form.

Providers will additionally attest to:

  • Average wait time for appointments
  • Availability of telemedicine services and direct patient care
  • Patient gender restrictions, if applicable.
