BlueCard Claim Appeal Member Authorization Requirements

An appeal of a BlueCard claim may be filed with the member's Blue Plan by the member or the billing provider. For the provider to appeal a claim denial on the member's behalf, the member must complete the Authorized Representative Appeal Form applicable to their Home Plan in accordance with their contract and the Home Plan's operating policy. The appeal can be submitted to either the member's Blue Plan or to BCBSKS. If submitted to BCBSKS we will forward to the member's Home Plan.
